These files were created by data@nber.org at the NBER to make access to the Centers for Medicare & Medicaid Services' (CMS) Healthcare Cost Report Information System (HCRIS) Health Clinic Cost Report Data files easier. Pre-FY2015 222-92 deliveries included both Federally Qualified Health Clinic (FQHC) and Rural Health Clinic (RHC) data. From 2015-2018 the Rural Health Clinics remained in the 222-92 deliveries, and then in form 222-17. FQHCs can be found in form, 224-14. Some materials are set up as well for the other HCRIS forms can be found in the following links. Please refer to the HCRIS page for the years that each form is released.

CMS provides HCRIS source data files and documentation for the 222-92 and 222-17 version of the files. The Provider Reimbursement Manual versions 222-92 and 222-17 files have an instruction manual and worksheets.

Select variables from the cost reports is available in the files below. One of the variables included is ZIP Code. A ZIP Code distance database is available.

Files can be located using the directory path of each download link replacing "https://data.nber.org/" with "/homes/data/".
