Earlier, pre-FY2015, Federally Qualified Health Clinic (FQHC) data was included in the 222-92 HCRIS Health Clinic data along with Rural Health Clinic (RHC) data. Starting 2018 the RHCs will can be found in the HCRIS RHC  222-17 deliveries, while the FQHCs will use the new form, 224-14.

CMS provides HCRIS source data files and documentation for the 224-14 version of the files. The Provider Reimbursement Manual versions 224-14 files have an instruction manual and worksheets.

This 224-14 worksheet materials are helpful for identifying the worksheet, line, and columns of the variables.

Select variables from the cost reports is available in the files below. One of the variables included is ZIP Code. A ZIP Code distance database is available.

NBER affiliates can access this and other data through the NBER computing system.  Files can be located using the directory path of each download link replacing "https://data.nber.org/" with  "/homes/data/". For example, these HCRIS datasets can be found under "/homes/data/hcris/" on the internal NBER computer cluster.
